Phoenix: posted cash next to the hospital's own network rates
Six of the ten services on the Phoenix sheet. The network rate is Banner Estrella Medical Center's rate for the named payer, from its machine-readable file. Labs and therapy are here on purpose: cash wins on access there, not price. 51 priced rows across 30 providers so far.

Questions operators ask
The plan funds the card. Payments to providers draw on the plan's funds. Rivendell never holds plan assets in its own account. The funding structure is set out in the written agreement.
Who we are
Rivendell is a cash-pay health plan and third-party administrator in San Francisco. We run our own level-funded plan on this rail in California and pay for our members' scheduled care at the counter.
Every price here was posted by the provider, confirmed by us on the phone, or is marked as reference only. The Cash Price rents that rail to other plans.
